Care Pathway

St George Healthcare Group have developed a Care Pathway which provides high quality care to meet the individual needs of the patient and includes an agreed way of working and a comprehensive set of policies and procedures from referral to discharge.

Pre Admission Assessment

• Pre admission assessment completed within 7-10 days of referral
• Objective baseline assessment undertaken
• Initial package of care designed and agreed with stake holders
• A comprehensive report is returned to the referring body

Our care pathway is supported by a strong multi-disciplinary and experienced rehabilitation team, who are committed to promoting the wellbeing and recovery of every individual

Initial Assessment

• Comprehensive assessment to meet the clients individual needs
• Rehabilitation package formulated by multi-disciplinary team
• Risk assessment undertaken
• Initial assessment report completed in 3-6 weeks

Rehabilitation and Treatment

• Individual rehabilitation package undertaken, focusing on cognitive, behavioural, emotional, physical, educational and or vocational needs as identified in initial assessment
• Rehabilitation model delivered by skilled clinicians and therapists using evidence-based interventions
• Objective evaluation of an individual’s progress conducted with a major review at 12 weeks

Discharge and Aftercare

• We commence discharge planning from admission, working closely with clients family and carers, in addition to external services and agencies
• We provide a strong after care plan and ongoing specialist support
• The key feature of the St Cyril’s care pathway is that it enables the service user to undergo a comprehensive assessment and rehabilitation programme within one hospital

Focus on Care, part of the St George Healthcare Group, can work with commissioners and local services to provide bespoke packages for discharge where appropriate.
