Initial Assessment
- Comprehensive assessment to meet the clients individual needs
- Rehabilitation package formulated by multi-disciplinary team
- Risk assessment undertaken
- Initial assessment report completed in 3-6 weeks
Rehabilitation and Treatment
- Individual rehabilitation package undertaken, focusing on cognitive, behavioural, emotional, physical, educational and or vocational needs as identified in initial assessment
- Rehabilitation model delivered by skilled clinicians and therapists using evidence-based interventions
- Objective evaluation of an individual’s progress conducted with a major review at 12 weeks
Discharge and Aftercare
- We commence discharge planning from admission, working closely with clients family and carers, in addition to external services and agencies
- We provide a strong after care plan and ongoing specialist support
