Challenging BehaviourSt Cyril's Rehabilitation Unit offers a specialist service to individuals who present a challenge to other services due to the complexity and severity of their behavioural and cognitive presentation, related to their neurological condition. The Team at St Cyril's have a wealth of knowledge and expertise in the assessment and rehabilitation of this patient group. Westminster Suite is a dedicated challenging behaviour service and is designed to offer a spacious low stimulus environment ensuring that rehabilitation potential is maximised. Patients may present with a range of challenging behaviours which require an intensive multi-interdisciplinary team approach. Our specialist Team at St Cyril's has the capacity to offer 24-hour care as well as an intensive individualised rehabilitation programme to address these problems, so the potential for progress in rehabilitation is maximised. Following an initial assessment by our specialist Team including neuropsychology and neuropsychiatry a Multi-disciplinary Report will form the basis of the evolving treatment plan and address the complexity of the cognitive and behavioural presentation which may have otherwise impeded further progress. Transitional Care is offered in order to provide a seamless pathway of care which includes the transition of rehabilitation achievements and interventions into a transitional package for patients returning to community living. This negates the need for multiple placements throughout the pathway of rehabilitation. Discharge planning at St Cyril's commences upon admission. Therefore we promote working in close partnership with families, commissioners, and external care co-ordinators to ensure their understanding, involvement, and collaboration at all stages of rehabilitation. |
